This guide explains how patients of different age groups can access their medical records online through the NHS system, either directly or via a proxy. A proxy is someone who is authorised to access a patient’s medical records on their behalf.
Ages 11–15 – Proxy Access
For patients aged 11 to 15, both the proxy and the patient must be registered at the same practice. To gain access, a request form must be completed at the practice. This access includes appointments, prescriptions, and Summary Care information only. Full clinical access is not available unless specifically approved by a GP. Access is subject to an annual review. After submitting the request, the practice will contact the proxy to confirm their identity and relationship to the patient.
Ages 11–15 – Own Access
Patients in this age group can also request their own access by completing a form at the practice. This allows access to appointments, prescriptions, Summary Care information, and full clinical records. Once approved, login details will be sent via email.
Ages 16 and Over
Patients aged 16 and above can register for access directly through the NHS App.
Ages Birth–10 – Proxy Access
For children aged birth to 10 years, a proxy can request access by logging into their NHS App and submitting a request. Both the proxy and the patient must be registered at the same practice. The practice will follow up to confirm the proxy’s identity and relationship to the patient.
Adult Proxy Access
Adults acting as proxies for other adults can also submit a request through the NHS App. As with other proxy arrangements, both parties must be registered at the same practice, and identity and relationship verification will be required after submission.
