Proxy and Patient Access Information
At our Practice, we provide different access options for patients and their designated proxies. Below is a breakdown of the access available based on the patient's age and the type of access requested.
A proxy is someone representing the patient in accessing their medical information. The patient is the individual whose medical record is being accessed.
11-15 Year Olds - Proxy Access

Proxy and patient must both be registered at our Practice.

Complete a request form at the Practice.

Available services: Appointments, prescriptions, and Summary Care Access only.

Full clinical access is not available unless specifically approved by the GP.

Access requires an annual review.

You will be contacted to confirm your identity and relationship to the patient after submitting your request.
Birth to 10 Year Olds - Proxy Access

Proxy and patient must both be registered at our Practice.

Log into your NHS App and submit a request through this service.

Adult Proxy Access (16 years and over)

Patient Access
11-15 Year Olds - Own Access

You will be emailed your login details after approval.
16 Years and Over - Own Access

Register via the NHS App.
Viewing and Requesting Your Medical Records
In addition to digital access, you have the option to request access to your medical records in writing, where you can either view your records with a member of our staff or request printed copies.
Viewing Records with Staff

You can book a 30-minute appointment with a member of our administration team to view your records in person.

Appointments must be scheduled in advance.
Requesting Copies
You may request copies of your medical records.

Please note, there is a printing charge of 10p per page - This is not a fee for accessing your records, but simply a charge for printing.

Alternatively, you may request documents to be emailed to you personally, this is provided free of charge.
