Care Homes We Work With

We maintain strong relationships with several local care homes, ensuring continuity of care and regular medical oversight for residents:

  • St Clare’s Care Home – Located just off Birchwood Avenue, St Clare’s offers residential and dementia care in a warm, community-focused setting. The home features en-suite rooms, a central courtyard, and a vibrant activity programme.
  • Swanholme Court & Eccleshare Court – These sister homes near Hartsholme Country Park provide residential and dementia care, with access to a hair salon, café, and daily activities.
  • Stones Place – A beautifully converted country manor offering residential and palliative care for older adults. 

Our practice team regularly liaises with care home staff to ensure residents receive timely medical reviews, medication management, and support with chronic conditions.

The Frailty Team

Birchwood Medical Practice is part of the Lincolnshire Frailty Strategy, which aims to identify and support patients living with frailty before crises occur. Our Frailty Team includes:

  • GPs and Advanced Care Practitioners
  • Occupational Therapists and Physiotherapists
  • Social Care Coordinators
  • Community Nurses

We offer Comprehensive Geriatric Assessments, personalised care plans, and proactive monitoring using tools like the Electronic Frailty Index (eFI). Our goal is to help patients remain independent, avoid unnecessary hospital admissions, and receive care in their preferred setting.
