Self-Certification (For Illness Less Than 7 Days)
If you have been unwell for 7 days or fewer, you do not need to see a doctor, you can Self-Certify. Please speak to your employer if they require documentation.
Sick/Fit to Work Notes (For Illness Over 7 Days)
If you have been unwell and unable to work for more than 7 calendar days, you may need a Fit Note (also known as a sick note) from a healthcare professional to give to your employer.
When You Need a Fit Note
- You only need a fit note if your illness lasts longer than 7 days.
- After 7 days, your employer may ask for a fit note to confirm your continued absence.
How to Request a Fit Note
- Contact the practice to request a fit note. This may be done via:
- An online form
- A telephone consultation
- A face-to-face appointment, if needed
- The clinician will assess your condition and issue a fit note if appropriate.
What the Fit Note Includes
- Whether you are not fit for work, or
- May be fit for work with adjustments (e.g. reduced hours, amended duties)
Fit notes can be issued by GPs, nurses, pharmacists, physiotherapists, or occupational therapists.
Returning to Work
- You do not need another fit note to confirm you are well enough to return.
If your job involves specific duties (e.g. driving, operating machinery), you may need additional checks before returning.
