Private and Professional Fees
Almost all of the services we provide are free of charge under the National Health Service. You will always be told if the service you request attracts a fee as recommended by the BMA. Payment by cheque or cash only.
Our current fees for the following services are:
Childminder application form (OFSTED) £30.00
Passport/Driving Licence Countersigning (Patient must be known to GP for more than 2 years) £15.00
Insurance claim forms (e.g. holiday cancellation, med insurance) £20.00
Letters “To whom it may concern” re. aspects of patient health/ private sick notes/fitness to travel etc. £15.00
Medical examinations and reports (e.g. HGV, employment) £80.00
Elderly drivers certificates £60.00
Yellow fever vaccination £40.00
Fitness to exercise certificate £10.00
