What the District Nursing Team offers
The
District Nursing Team delivers professional nursing care to patients in their
own home.
All
aspects of health care are covered and the district nursing team may work in
conjunction with many other professionals in caring for people.
Provision of care includes caring for people who cannot visit the surgery with,
for example, many varieties of wounds, incontinence or people who have
catheters.
The
team undertakes complex assessment of health needs with referral to other
agencies, if indicated by the assessment.
The
District Nursing Team members undertake regular updating studies and are the
referral point for incontinence, leg ulceration and palliative care for patients
who have a cancer (usually in conjunction with the MacMillan Nurse Specialist).
We are also actively
involved in training student nurses from Lincoln and Nottingham Universities.
This provides the students
with a valuable placement, gaining understanding of our patient's needs and the
role of the district nursing team within the community.
